A local Dementia Community Support Team has been shortlisted for the ‘Team of the Year’ in the coveted Local Government Chronical (LGC) awards, which recognise the best of local government innovation and service delivery.
The team who jointly work on behalf of Southend-on-Sea Borough Council and the NHS across south east Essex support local residents affected by dementia to make sure they get the support they need while ensuring the local area is as dementia-friendly as possible.
Jacqui Lansley, Director of Integration and Partnerships for NHS Southend Clinical Commissioning Group (CCG) and Southend-on-Sea Borough Council said:
“This is an absolute testament to the work that this team has carried out as part of our Integrated Commissioning approach working in partnership with GP practices, the hospital and our Community Services. They thoroughly deserve to be shortlisted for this award.”
Dr Jose Garcia Lobera, local GP and Chairman for NHS Southend CCG and Dr Kashif Siddiqui, local GP and Chairman for NHS Castle Point and Rochford CCG said: “A diagnosis of dementia can have a big emotional, social, psychological and practical impact on a person. It is important that our local residents affected by dementia are able to get the right support and advice so that they can live well with the condition, based on that understanding. The compassion and dedication shown by the team to support local people and make the local area, businesses and healthcare settings dementia-friendly is inspirational.”
Dementia Navigators form part of the local dementia support teams, they support local residents and their family throughout the dementia experience, offering support and guidance to help you understand dementia and the day to day challenges it may bring. The service is available to people pre-diagnosis as well as post diagnosis and provides the crucial link between the person with dementia, their carer and health, social care and community support. Since the service went live in April 2017, over 700 referrals have been received.
